WHY SHOULD I APPLY FOR THIS PLAN?

Accidents and illness occur with no warning. What would happen to your family if you became injured or sick and could not work? The last thing you would want to worry about is how your bills were going to be paid. You can help prevent this anxiety by considering your association endorsed plan. A plan that is extremely economical due to the mass purchasing power of the Association.

HERE'S HOW THE ASSOCIATION ENDORSED PLAN WORKS.

Your plan will pay a monthly benefit amount to a maximum of $7,500 but not to exceed 60% of your monthly-earned income less other disability income insurance in effect. You may choose a plan that pays monthly disability benefits to Age 65 or for 5 years for an accident or sickness as long as you remain totally disabled. Total disability means you will receive own-occupation protection for up to five years for whichever plan you choose. This means you qualify for benefits if you cannot perform the substantial and material duties of your own regular occupation. Thereafter, the definition of disability is the continuous inability to perform any occupation for which you are reasonably suited in terms of education, training and experience. Own occupation means your specialty in practice of medicine or dentistry.

AGE 65 PLAN

Benefits Payable For Total Disability beginning:

Before Age 63: To Age 65
Age 63 - 69: 2 years

5 YEAR PLAN

Benefits payable for Total Disability beginning:

Before Age 61: 5 years
Age 61: 4 years
Age 62: 3 years
Age 63-69: 2 years

You may also choose from one of these waiting periods:

PLAN A 90 days for injury or sickness
PLAN B 180 days for injury or sickness
PLAN C 365 days for injury or sickness

You will begin to receive monthly benefits after your waiting period ends.

THE PLAN CAN HELP YOU WHEN YOU BECOME PARTIALLY DISABLED.

Your benefits will continue even if you return to work. If you are unable to work for more than four hours a day and are under the care of a physician, this plan will pay for up to six months at 50% of your total disability amount. Benefits begin on the first day following the last day when Total Disability benefits have ceased.

THE PLAN CAN HELP WITH REHABILITATION.

This plan places its emphasis on abilities rather than disabilities. A staff of nurses and trained counselors will custom tailor a program to fit your needs and your aspirations so you have a better and more productive life. We'll work with you to get you back on your feet.

WILL MY PLAN HAVE A WAIVER OF PREMIUM PROVISION?

Yes - Future premiums will be waived for as long as benefits are payable after you have been totally disabled for six months.

WILL MY PLAN HAVE A SURVIVOR BENEFIT?

Yes-If you die while receiving disability benefits, your beneficiary will receive a lump sum benefit worth three times the last monthly benefit period.

HOW DO I KNOW IF I CAN APPLY FOR THIS PLAN?

All members under age 60, who are actively at work full-time for at least 30 hours per week, live in the United States.
